LiveSafePro Post-Acute Care Utilization Management
In the post-acute environment, many seniors require medical care in order to recover lost function. They may be frail or incapacitated as a result of injury and/or illness such as pneumonia, stroke, or a fracture. SeniorMetrix has established leading-edge technological capabilities for assessing patients, identifying the correct care setting, establishing the intensity/duration of care, and projecting the expected outcome.
LiveSafeHome Community-Based Care Management
SeniorMetrix provides a suite of solutions for improving the overall identification and care management of frail members who live in the community. Specifically, many members have diminished functional capabilities due to disease, injury or the aging process, and they need care on a daily basis. Using the SeniorMetrix technology, health plans are able to objectively assess a member’s needs, establish a safe care plan, and monitor results.

Our History
Founded in 1999, SeniorMetrix partners with health plans and companies serving Medicare Advantage and Medicaid members. Known as the pioneer of using post-acute data to reduce practice variation, the company’s proprietary database uniquely combines both clinical outcome and utilization data. The magnitude of the data is continually enhanced by nurses or rehab therapists who have demonstrated their ability to accurately collect clinical data elements. Headquartered in Brentwood, Tennessee, the company works with some of the largest health plans across the country, representing 900,000 Medicare Advantage and 3,000,000 commercial lives.
Most recently, SeniorMetrix introduced a new product line called, LiveSafe. This new platform is a leading-edge technology that empowers the use to leverage the power of functional measurement with over ten years of rehabilitation and burden of care outcomes.
Specifically, LiveSafe technology uses Item-Response Theory to objectively evaluate an individual’s level of function. The user responds to assessment questions related to daily living, mobility and cognition. LiveSafe technology combines the individual’s functional status with the SeniorMetrix database of actual outcomes to establish evidence-based criteria for determining a care and treatment plan for older adults recovering from a serious illness or injury.
Within the LiveSafe suite of products are LiveSafeHome for Managed Medicaid Plans and LiveSafePro for Medicare Advantage Plans.
