OPT - Services
The OPT tool has variations that focus on different aspects of post-acute planning. Each version provides a real-time prospective about:
- The least intensive setting in which a patient can safely be discharged
- How many days or visits will be required for a safe discharge
- How much rehabilitation should be delivered each day
The information that an OPT tool provides can be used by discharge planners, case managers, therapists and physicians to enhance the delivery of treatment on a case by case basis.
The tools have demonstrated their effectiveness in both onsite and telephonic case management systems. The one-time training in the use of the tool also helps health care professionals communicate more clearly about patient’s needs through the use of a common metric and language.
