NASHVILLE, Tenn. – November 25, 2003 — SeniorMetrix, the leader in predictive modeling and quality management for post-acute care, and GeriNet, Southern California’s premier post-acute medical specialty practice, have entered into an agreement to combine the strengths of both firms. The goal of this relationship is to identify opportunities for improving the delivery of post-acute care for Medicare + Choice enrollees in Southern California.
GeriNet, through the provision of post-acute medical services for medical groups, will begin to utilize SeniorMetrix’ extensive database of clinical outcomes to establish more realistic goals for patients and to monitor their functional change as they receive physical rehabilitation services. The goal of this approach is to improve quality through the reduction of practice variation.
As a patient enters a skilled nursing facility, GeriNet physicians and Registered Nurse Practitioners will have access to functional improvement, discharge setting, therapy intensity and length of stay information for a sample of previous patients who are very similar based on reliable matching characteristics. “This clinical data will give patients, family members, therapists and primary care physicians a much more accurate idea of what to expect during and after the patient’s post-acute episode,” said Kim Phan, CEO of GeriNet.
